Register as New Patient
Patients living within our practice area (please see link below) are welcome to register. Please ask the receptionist for a registration pack or print off and complete the forms below and bring them into the surgery with the necessary ID etc.
When you register you will be registered with a named GP. Please see the welcome letter for more details.
Medical treatment is available from the date of registration but where possible please allow 24 hours before booking an appointment etc to allow us to process your application.
The link below shows our inner and outer boundary. The blue line represents the inner boundary, this is the area we accept new patients from. The red line indicates our outer boundary, if you move house to an area within this red boundary you can continue to be registered with Hillview Medical Centre, if you wish. If you move to an area outside this red boundary you will be requested to register with a surgery closer to your home.

Temporary Patient Registrations
If you are ill while away from home or if you are not registered with a doctor but need to see one you can receive emergency treatment from the local GP practice for 14 days. After 14 days you will need to register as a temporary or permanent patient.
You can be registered as a temporary patient for up to three months. This will allow you to be on the local practice list and still remain a patient of your permanent GP. After three months you will have to re-register as a temporary patient or permanently register with that practice.
To register as a temporary patient simply contact the local practice you wish to use. Practices do not have to accept you as a temporary patient although they do have an obligation to offer emergency treatment. You cannot register as a temporary patient at a practice in the town or area where you are already registered.
